Physical Form and Medical History

In order to participate in middle and high school athletics, including tryouts, all students are required to have a current physical on file. A physical is considered current if it was given on or after April 15 of the previous school year. To submit your sports physical, please use FinalForms.

The Athletic Department of Northville Public Schools has collaborated with FinalForms, an online service for managing forms and data. With FinalForms, parents can complete and sign athletic participation forms for their students. We urge all parents of athletes from Northville High School, Hillside Middle School, and Meads Mill Middle School to register their children on FinalForms. During registration, please indicate which sports teams your athlete is interested in to receive communication about the upcoming season.

Pay to Participate 

All students participating in athletics, grades 6 -12, will pay a one-time yearly fee.  There is a $770 per family maximum
 
  • Middle School (6th) $100/player
  • Middle School (7th - 8th) $200/player
  • High School (9th - 12th) $385/player
 
**Please note that the pay-to-participate fee does not apply to the following high schools club teams: Alpine Ski, Bowling, Competitive Dance, Equestrian, Field Hockey, Figure Skating, Girls Hockey, JV Boys Hockey, Lacrosse, Mountain Biking, Rowing, and Boys Volleyball. These teams will have their own separate fees for the season.  
 

Payment Options**  

  • Complete the Pay to Participate form provided below and send it along with a check to your school. Payment is due by the 2nd week of the start of the season.
  • Pay by credit card at NorthvilleSchools.RevTrak.net (an additional credit card processing fee will be added to the credit card).
 
*To avoid any unnecessary expenses, we request that the pay-to-participate fee for cut sports be delayed until the athlete has officially made the team

Middle School Athletics

Because of restrictions by the Michigan High School Athletic Association, the governing body of athletics for middle schools and high school in the state of Michigan, interscholastic athletic competition is only available for 7th and 8th graders. However, there are many activities that 6th graders may participate in and some of these are athletic.

 

Questions should be directed to the coach/sponsor first and then the assistant principal who is in charge of student activities.
