Physical Form and Medical History

All students must have a current physical on file to participate in middle school and high school athletics including tryouts. A current-year physical is one given on or after April 15 of the previous school year.  Submit your sports physical through FinalForms.  
 
 

FinalForms

Northville Public Schools Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students.  We are asking that ALL parents of Northville High School, Hillside Middle School and Meads Mill Middle School athletes register their athletes on FinalForms. Please check off all sports teams your athlete is interested in to receive communication for the upcoming season.

Pay to Participate 

All students participating in athletics in grades 6 -12 will pay a one-time yearly fee.  There is a $770 per family maximum. (**Fee does NOT pertain to Alpine Ski, Bowling, Competitive Dance, Equestrian, Figure Skating, Hockey/Girls, Hockey/JV Boys, Lacrosse, Mountain Biking, and Rowing)  
 
  • Middle School (6th) $100/player
  • Middle School (7th - 8th) $200/player
  • High School (9th - 12th) $385/player
 
Payment Options**  
  • Fill out the Pay to Participate form below and mail it along with a check to your school. Payment is due by the 2nd week from the start of the season.
  • Pay by credit card at NorthvilleSchools.RevTrak.net (an additional credit card processing fee will be added to the credit card).
 
**If your athlete is trying out for a cut sport, please do not pay until the athlete has made the team.

Middle School Athletics

Because of restrictions by the Michigan High School Athletic Association, the governing body of athletics for middle schools and high school in the state of Michigan, interscholastic athletic competition is only available for 7th and 8th graders. However, there are many activities that 6th graders may participate in and some of these are athletic.

 

Questions should be directed to the coach/sponsor first and then the assistant principal who is in charge of student activities.

Parent Survey

The purpose of this survey is to obtain information and comments from student-athletes and parents regarding the recent sports season that would help improve the athletic programs offered by the Northville Public Schools. We plan to periodically survey student-athletes and parents following each season.
